Greetings,

Health Services has created an electronic version of the Student Health Information form. Due to COVID-19, the health office will not be sending out paper copies.

Please complete this survey if your student is new to the Clark County School District or there have been any health related changes.

If the reported health condition(s) of your student has not changed there is no need to complete the survey at this time. Please click on the provided link to complete the survey:
