Immunizations for 7th Grade

Mannion Middle School - Home of the Mustangs

Students enrolled in 7th grade must show proof of two additional immunizations:  Tdap & MCV4.  7th grade students will not be permitted to start school unless a copy of the immunization is received.  Please provide a copy of the immunization at least 1 week before the start of the school year.

All immunization records may be submitted below, including annual religious and medical exemption certificates.
