Follow these steps if your student is transferring from a CCSD School
Students transferring to Mannion Middle School because they moved to our zone, received a zone variance, or were accepted as part of open enrollment must bring the following documents to enroll at Mannion Middle School.
- Document One: Proof of Address
- The parent/legal guardian must provide one document in their name as proof of address. Acceptable forms of proof of address are:
- Recent utility bill (gas, power, or water)
- Receipt of Rent
- Residential lease or sales contract
- Residential Affidavit (temporary living condition with no proof of address). This form, which must be both completed and notarized, is available in English by clicking here and Spanish by clicking here.
- Examples of UNACCEPTABLE documents for proof of address are:
- Driver’s License
- Telephone Bill
- Cable Bill

- Document Two: Proof of Identity
- Parents/guardians must provide a proof of identify to register a child for school.
- Parent/Guardian – Proof of Identity (1)
- Driver’s License
- Passport
- Parent/Guardian – Proof of Identity (1)
- Parents/guardians must provide a proof of identify to register a child for school.
- Document Three: Withdrawal Paperwork from Previous School
- Students must be withdrawn from the previous school in order to be enrolled at Mannion Middle School.
