Follow these steps if your student is transferring from a Non-CCSD School
Students new to CCSD must begin the Online Registration Process (OLR) through the CCSD website. This process can begin at a home computer with internet access or at an available school computer if internet access is not available at home.
Step One
Complete the Online Registration Process (OLR) through the CCSD website at register.ccsd.net. The OLR process will require the following information:
- Primary Household Information
- Contact Information
- Home Address
- Mailing Address
- Parent/Guardian Information
- Secondary Households
- Impact Aid
- Emergency Contact Information
- Only 4 emergency contacts are allowed per student
- Other Family Members
- Step Parents and siblings not of school page
- Student Information
- Race/Ethnicity, Housing (Title I Hope), Student Services (Special Programs)
- Language Survey, Tribal enrollment
- Release Agreements: Directory Release, Media & Public Release, Computer Network Access, Code of Honor, Educational Accord, Electronic Transfer of Grades
- Transportation
Step Two
Once the OLR Process is complete, the documents listed below will need to be provided to Mannion Middle School in order to complete the registration process. The required documents must be brought to the Mannion Administration Office.
- Document One: Proof of Address
- The parent/legal guardian must provide one document in their name as proof of address. Acceptable forms of proof of address are:
- Recent utility bill (gas, power, or water)
- Receipt of Rent
- Residential lease or sales contract
- Shared Housing Application (temporary living condition with no proof of address). Additional information about Shared Housing is available here by clicking here.
- Examples of UNACCEPTABLE documents for proof of address are:
- Driver’s License
- Telephone Bill
- Cable Bill
- The parent/legal guardian must provide one document in their name as proof of address. Acceptable forms of proof of address are:
- Document Two: Proof of Identity
- Parents/guardians must provide a proof of identify to register a child for school. Proof of identity is required for both the parent as well as the student.
- Parent/Guardian – Proof of Identity (1)
- Driver’s License
- Passport
- Student – Proof of Identity (1)
- Certified Birth Certificate
- Parent/Guardian – Proof of Identity (1)
- Students must be enrolled in school under the name EXACTLY as it appears on the identifying document unless the parent or legal guardian provides a court order or decree authorizing a change of name.
- Parents/guardians must provide a proof of identify to register a child for school. Proof of identity is required for both the parent as well as the student.
- Document Three: Immunizations
- Parent/guardian must provide written proof that immunizations are up-to-date and compliant at the time of enrollment.
- Students will not attend class until proof of immunization compliance is provided, unless the conditions for medical or religious exemption are met.
- A grace period for immunizations is not allowed, unless the student is of a military family (30 day grace period to obtain immunization records or to initiate the required immunization series).
- Students enrolled in 7th grade must show proof of Tdap and MCV4 immunization. Please click here for additional information.
