Vaccination Information
Students entering Grade 7 or Grade 12 throughout Nevada are required to receive the meningococcal vaccine. The meningococcal vaccine is an immunization against meningitis. Students entering Grade 7 are also required to receive the tetanus, diphtheria, and pertussis (Tdap) vaccine. The requirement, approved by the State Board of Health as a State Regulation, is for all public, private, and charter schools in Nevada. Students may not be permitted to attend school without these vaccinations documented or with approved exemption.

Vaccination Exemptions
All exemptions of student immunizations for religious beliefs must be submitted on the State of Nevada Preschool–12th Grade–Religious Immunization Exemption Certificate. This form must be completed and submitted annually to school offices.
